Introduction to Drug Interactions

Today, we will answer an important question: Does this drug interact with another drug? If you are asked about the possibility of taking two drugs together, or what is known as drug-drug interactions, how do we verify that?
Practical Example (1): Tamoxifen and Paroxetine
Let's apply a practical example; a woman in her forties was diagnosed with breast cancer two years ago, and since then she has been treated with the drug "Tamoxifen". She has been suffering from hot flashes and depression for weeks, do you think that the drug "Paroxetine" is suitable for her?
Steps to search in the Medscape app:
- Open the app and choose the fifth option: Drug Interaction Checker.
- Click on it and type the names of the two drugs in question: (Tamoxifen) and (Paroxetine).
- The result shows that there is one drug interaction (1 Interaction).
The Problem: The app indicates that "Paroxetine" inhibits the conversion of "Tamoxifen" into its active form, and therefore the patient will no longer benefit from the cancer treatment.
Recommendation: The patient should be closely monitored if both drugs are given together, but in this case, it is preferable to avoid combining them.
Practical Example (2): Ranolazine and Diltiazem
Let's apply another example; a patient suffering from angina or reduced blood flow to the heart muscle, and we want to see the interaction of two drugs used together.
- We remove the previous drugs and write the new ones: "Ranolazine" and "Diltiazem".
- The result shows one conflict as well.
- Recommendation: Closely monitor the patient (Monitor Closely), and the app recommends that if they are combined, the dose of "Ranolazine" should not exceed 500 mg twice a day.
This is how you can search for drug interactions for any two or more drugs; you can type three or four drugs and the app will give you all the interactions between them.
